Duwār (Vertigo)

In Duwār the patient feels that their surrounding objects are spinning. He also feels that their body and brain are spinning.

Etiology: The Khām Ghalīz Mawād (Immature concentrated matter) is the morbid matter of this disease that abnormally accumulates in brain, on heating it produces thick gases which stimulates Rūh Nafsāniyya (Psychic pneuma) leads to symptom of Duwār.

Symptoms: sensation of spinning of head and objects, darkness in front of eyes, heaviness of head and tinnitus. These symptoms appear while standing and the patient falls down due to imbalance.

Investigations:

1.     Tafsira e bole

2.     Examination of nebz

3.     Complete Blood Count

4.     X-ray Cervical Spine.

Usūl-e Ilāj (Principles of treatment):

       I.            Tanqiya-e Dimāgh (Evacuation of morbid matter from brain)

    II.            Tanqiya-e Meda (Cleansing of stomach)

 III.            Tahlīl-e Riyāh (Resolution of flatulence)

IV.            Taltīf-e Ghizā (Dietary modulation)

   V.            Taqwiyat-e Dimāgh (To tone-up the brain)

Ilāj bil-Dawā (Pharmacotherapy): Mufrad Adia:

1.     Shīra prepared with Bādām 5 pieces, Khashkhāsh 9 gm., Kishnīz 9 gm. mixed with 36 gm.of sugar  and sprinkled with 7gm.of Tukhm-e Faranjmushk for oral administration.

2.     Joshānda-e Aftīmūn, orally in case of predominance of black bile.

Drug of choice (Murakkab Advia):

1.     Ayārij Fayqra 3-5 gm.

2.     Habb-e Qawqāya 4.5 gm. with lukewarm water at bed time

3.     Murabba  Seb

4.     Itrīfal Kishnīzi 9-24 gm. at bed time

5.     Itrīfal Aftīmun 5 gm.

6.     Anūshdārū Lūluwi 7 gm

7.     Jawārish Tabāshīr 5-7 gm with Arq Gāozabān.

8.     Khamīra Gaozaban Ambari jadwar ood saleeb wala 3-5 gm.

9.     Mā ul-Lahm 108 ml.

Ilāj bil-Tadbīr (Regimenal therapy):

1.     Fasd-e Qīfāl (Bloodletting through cephalic vein)

2.     Hijāma (Cupping) on the nape of neck and calf muscles.

3.     Ta‘tīs (To induce sneezing)

4.     Qay’ (Emesis) in case of gastric involvement

Dietary recommendations:

1.     Aghziya Latīfa

2.     Aghziya Musakhkhina in case of predominance of Burūdat (Cold)
